Statement.

 

Convicted A.K.S. is serving her sentence in the correctional institution of the Russian State Institution "Institution No. 10" of the Ministry of Internal Affairs of the Republic of Kazakhstan in the village of Zhaugashty, Iliysky district, Almaty region..

According to the certificate of health dated 04/01/2024 of the Ili Central District Hospital of A.K.S., the diagnoses "Bronchial asthma, persistent of moderate severity, partially controlled. Day 0 ct", "Arterial hypertension of the 3rd degree, risk of 3".

These diseases are included in the list of diseases on the basis of which a convicted person can be submitted for release from serving a sentence due to an illness, according to the Order of the Minister of Health of the Republic of Kazakhstan dated June 30, 2022 No. KR DSM-58 "On approval of the rules for conducting a medical examination of convicts submitted for release from serving a sentence due to an illness, and the list of diseases that are the basis for release from serving a sentence"

Based on the above, guided by paragraphs 2, paragraphs 5-10 of these Rules, I ask:

- to send convicted A.K.S., born on 12/13/1969, IIN, for examination by a special medical commission ....

Appendix: a copy of the health certificate of A. K.S., a copy of the notice of protection (representation), a copy of the lawyer's certificate, a copy of the power of attorney, a copy of the certificate of state registration of a legal entity.
