1. The applicant submits to the accreditation body the following documents on paper or in electronic form through the information system of technical regulation:

1) an application for accreditation;

2) notarized copies of documents establishing the applicant's legal status;

3) the claimed area of accreditation;

3-1) Quality manual;

4) passport for the declared field of activity in the field of conformity assessment;

5) information about personnel performing conformity assessment work for conformity assessment bodies;

6) regulations on structural divisions and their structure.

The applicant, depending on the regulatory document chosen by him, provides relevant information.

All paper documents must be submitted in duplicate, with the exception of the application for accreditation.

2. The forms of the documents specified in subitems 1), 3-1) and 4) of the first part of paragraph 1 of this Article shall be established by the authorized body.

3. If the application and/or the documents attached to it do not correspond to the prescribed form or are not submitted in full, as well as if the claimed equipment is used by another accredited laboratory, the accreditation body refuses and returns the documents to the applicant within five working days from the date of their receipt, indicating the reasons for the return by mail. either he hands them over to the representative on purpose against signature, or sends them in electronic form through the information system of technical regulation.

This Law regulates public relations in the field of accreditation of conformity assessment bodies, as well as related relations.
