In accordance with subparagraph 2), 3), 6) of paragraph 3, Article 33 of the Law of the Republic of Kazakhstan "On Advocacy and legal assistance", in order to provide qualified legal assistance to convicted A.K.S., I ask you to provide information (information):
A certificate of the state of health of the convicted A.K.S. – complaints, medical history, presence of diseases, recommendations of specialists, etc.
A.K.S. is serving a custodial sentence in the correctional institution of the Russian State Institution "Institution No. 10" of the Ministry of Internal Affairs of the Republic of Kazakhstan in the village of Zhaugashty, Iliysky district, Almaty region.
Appendix: a copy of the notice of protection (representation), a copy of the lawyer's certificate, a copy of the power of attorney, a copy of the certificate of state registration of a legal entity.
