Individual Entrepreneur “DA”, represented by I Z.A., having considered the request of the Senior Inquiry Officer of the Inquiry Department of the Police Department of the Almaly District of Almaty, Senior Lieutenant of Police Nazira U.B., in the criminal case registered in the Unified Register of Pre-Trial Investigations under No. 257511031000798 dated April 18, 2025, hereby informs that the persons specified in the request, namely V A.A. and G A.A., have never been in employment or civil-law relations with IE “DA”, have not been listed on the staff of the organization, have not been engaged in medical activities within the framework of IE “DA”, and have not provided any services on behalf of or upon the instructions of IE “DA”.
In connection with the above, IE “DA” does not have medical documents concerning the treatment received at IE “DA” for the entire period starting from February 2024 in respect of the persons specified in the request, due to the fact that medical services were not provided to these persons at IE “DA”.
Thus, it is not possible to provide the requested documents for objective reasons, due to their absence.
