Statement

(on establishing the fact of a citizen's death)

From _______ (date) I am in a registered marriage with ____________ ( Full name, year of birth), which I confirm with a copy of the marriage certificate. We have minor children from a joint marriage: a son - ____________ ( Full name, year of birth), daughter - ____________ ( Full name, year of birth). __________ (date) my spouse – ___________________ ( specify full name, date, month, year of birth) died, which can be confirmed by witnesses. In order to apply for a survivor's pension, I need to obtain a death certificate. The registry office refused to issue a death certificate, arguing that there was no medical certificate. An application for establishing the fact of a citizen's death On the basis of the above, guided by subparagraph 8) of the second part of Article 305 and Articles 306-308 of the Civil Procedure Code of the Republic of Kazakhstan, Paragraph: To establish the fact of a citizen's death _____________________ ( F.I.O., date, month, year and place of birth of the deceased spouse), the day of death is considered ____________ ( date, month, and year).
